MEMBERSHIP TERMS AND WAIVER

By submitting this online application, you agree to abide by The Moms Cooperative Bylaws, Membership Terms, and Code of Conduct and any and all corporation policies and guidelines.

You also acknowledge that you are a voluntary member of the organization and local group (in person/online).

By submitting and electronically signing this application, you give permission for yourself and any family member to attend and participate in any corporation events and local groups (in person and online).

By agreeing to join The Moms Cooperative Corporation or any of its groups, you agree to allow the corporation to connect you electronically with others who have joined Corporation, by sharing with them your contact information we have on file such as group name, name, email, phone.

This waiver shall remain in effect for each year you are a member of The Moms Cooperative.
